Fees & Insurance
Group session fee
$50-60 per group session (per participant)
We accept cash, all major credit cards, and HSA as forms of payment.
You may also send payment through our business PayPal account or bring in payment at time of your session.
If you are unable to attend a session, please cancel at least 24 hours before the start of your appointment. If you miss your session without notifying the provider, you may be charged a no-show fee.
NOTE: PAYMENT IS DUE ON THE DAY OF OR DAY BEFORE SERVICES ARE RENDERED. (e.g. Insurance copay or full fee for self pay)
We are In-Network (INN) with the following:
Caresource Marketplace (Commercial)
We accept self-pay (out of pocket) and commercial insurance.
Tricare/Humana (Approved Provider yet considered Out-of-Network)
You may use your insurance benefits Out-of-Network (ONN).
ONN is for insurance plans that we are not "In Network" with, yet they may reimburse for our services.
If you have insurance that we do not take, we will provide you with a SuperBill/Invoice. You can submit this to your insurance company to ask for direct reimbursement of services rendered.
We do not accept any form of Medicare/Medicaid/HIP at this time.
